If the effects disappear or seem to decrease after a good period, it may mean that the optimal dose for the patient has been exceeded.
Cannabinoid receptors can become dysfunctional if too much cannabinoids are introduced into the system * .
We need about 1-3 weeks to see the full results of previous dose increases - so if we increase too quickly, we won't see the full results before we add again.
So after a while it accumulates and e.g. in autistic children – who are very sensitive – we see that an excess of even half a mg can throw them out of the therapeutic window (the dose range in which CBD is therapeutically effective). So let's start very low.
If there is NO result with one dose (NOTHING at all, even the smallest positive change counts as a result), then we can increase the dose and follow the same process.
If there is any change - then we wait on that dose - at least three weeks or more until we get a very good idea of how that particular dose is working. (In severe cases, the dose can be increased more quickly). Then we increase the dose further - using the same procedure.
If we do not get a better result after the added dose - we return to the previous dose. If the positive effects stop, restart the dosage (two days of oil-free - or more) and start again at the dose with which you first had good results, depending on how long you used the dose that affected the receptors "saturated".
*The so-called restart or wash out can help re-sensitize the receptors. For example, after 3 weeks of using CBD oil, a 4-5 day rinse. During this period, we completely suspend the use of CBD oil. The restart can be started with a much lower dose, even half as much, because in this case the receptors will be much more sensitive, so the same effect can be achieved with a lower dose.
Example:
Someone uses an oil containing 3 mg/drop of active ingredient and increases it every 3-4 days. By the time the body is saturated, it may be several drops beyond the point that was ideal, so you may have to return to a much smaller dose. You can always return to larger doses slowly and carefully. This way we can find the "limits of the therapeutic window".
Let's say someone receives 11 mg and slowly increases (1 mg/day). When we see the first signs of a decrease or deterioration of the therapeutic effects (say at 20 mg), then we know where the therapeutic range is, within which to experiment. We find that if we increase the dose and there are no better results with the new doses, we can go back to the previous level and stay there. Chances are there will be the right dose somewhere.
Example:
The subject has been receiving a dose of 16 mg/day for a long time and his condition is starting to deteriorate. So the body was getting just a tiny bit more with each dose than it needed and it took a long time for those many small extra doses to build up in the body, so the dose should probably be brought back to 14 or 15 mg.
If someone increases too quickly, they may see the current good effect due to a previous lower dose, which is why they have to wait longer with 1-1 doses.
According to a medical observation, after restarting, it is possible to start again with half of the last working dose and even achieve better results.
